4. Information Provided on Behalf of Children

Physio Plus Tech does not collect information from children under the age of 13. If you are a parent or legal guardian of a minor child, you may, in compliance with the Terms of Use the Site or Services on behalf of such minor child. Any information that you provide while using the Site or Service on behalf of your minor child will be treated as Personal Information as otherwise provided here.

We share certain categories of information we collect from you in the ways described in this Privacy Policy, including as described below: If you elect to complete a Basic Medical Data form using the Services in advance of an appointment, we may share your Basic Medical Data with your doctors. We may share your Contact Data, Demographic Data with doctors you choose to schedule on the portal.

We will display your Personal Information and Member Content contained in the patient or family member profiles created by you via our services according to the preferences you set in your account. Any information you choose to provide and make available to third parties via our services should reflect how much you want others to know about you and the individual(s) about whom you created the profiles and accounts.

Please consider carefully what information you disclose in the patient profiles and your desired level of anonymity. You can review and revise the information contained in your profiles at any time. We recommend that you guard your sensitive information. Moreover the user who is taking the appointments on your behalf as well as the assigned CARETAKER will receive the information and can contact the provider to get perspective on the patient's health.

6. Confidentiality and Security

Except as otherwise provided in this Privacy Policy, we will keep your Personal Information private and will not share it with third parties, unless we believe in good faith that disclosure of your Personal Information or any other information we collect about you is necessary to:

(1) comply with a court order or other legal process;

(2) protect the rights, property or safety of Physio Plus Tech or another party;

7. Security

The security of your Personal Information is important to us. We follow generally accepted industry standards to protect the Personal Information submitted to us, both during transmission and once we receive it. We have no obligation to delete any data or information you provide on a Medical History Form following termination of your account or the account of any medical professional to whom your Medical History Form was submitted through the Site. 9. Lost or Stolen Information
